Who Is InCommunity?
InCommunity, a 501 (c) (3) non-profit agency headquartered in Atlanta, GA, provides community-based services and meaningful support to individuals with intellectual and developmental disabilities to live safe, healthy, and joyful lives in their communities.
Our offerings include residential support, employment services, respite care, family support, in-home care, and day programs. To date, InCommunity has provided support and service to more than 10,000 Georgian community members.
Our History
In 2014, enAble of Georgia and Georgia Community Support and Solutions (GCSS) joined forces to provide quality support to people with developmental disabilities. The long-term strategic planning process culminated as the two organizations started combining efforts, merging operations, and providing an even greater set of services. The coming together of these two purpose-driven teams resulted in an exciting new organization with a new name and brand: InCommunity.
The name, InCommunity, describes our mission as partners dedicated to enriching and sustaining the lives of individuals who have disabilities, their families, and their communities. We believe in the importance of the common good to wholly transform individual lives and collective communities. This passion for embracing the abilities in all of us, drives us to support community members with activities and experiences that make life purposeful such as career training, education, fitness programs, artistic exploration, and spiritual pursuits. Activities that provide self-esteem, growth, friendships, and love.

Our Programs and Services
Adult Day Programs
Adult Day Programs provide opportunities for adults with intellectual and developmental disabilities to participate in meaningful activities within their communities. Locations include: Marietta, Conyers, South Atlanta, and Roswell.
Family Care Programs
Maintenance and Emergency Respite are also available. Maintenance Respite is 30 days with subcontracted providers that go into the private homes of families or utilize their own homes as the provider facility. Emergency Respite is out-of-home care available from 1 to 6 weeks and requires a referral/approval from the Georgia DBHDD region.
Family Support Funding
Family Support Funding assists families in obtaining goods and services necessary to ensure the best possible care for their loved one.
Residential Options
Residential Options supports interdependence for individuals to live on their own terms in a living arrangement that supports their individual needs. It includes small group settings of four or less in HOST home/life sharing arrangements.
Employability (Supported Employment)
EmployABILITY provides job-seeking and employment support for post-high school adults with developmental disabilities to seek, secure and train, and maintain competitive employment in their community through Supported Employment (SEI) Services.
Community Living Support
Community Living Support provides two options: Individually tailored assistance for individuals who reside in their family home; and one to one teaching/coaching sessions on independent life skills in the individual's home.
Behavior Support
Behavior Support works with both family and individuals to establish specific goals and objectives for different types of behaviors.
Crisis Support
Crisis Support services offer intensive in-home support for up to 7 days, including operating across four crisis homes specifically catering to individuals with developmental disabilities and Autism Spectrum Disorder (ASD) who are facing severe behavioral challenges.
Day Programs
Art & Food
Art & Food provides training and skills to individuals who are interested in the art and culinary service industries along with community inclusion activities such as Dave & Busters, mini-golf, and more!
Beyond Horizons Center - BHC
BHC keeps our participants involved in as many community activities as possible. Partners include: College Park SkyHawks Basketball Team, Stars & Strikes Bowling, and Kennys’ Kreams’ Ice Cream Shop, and more!
Unlimited Possibilities Center - UPC
UPC provides therapeutic activities, social interaction, and community activities such as movie theater outings, bocce ball, look-and-cook classes, and gardening at Lanier Community Garden.
Marietta Enrichment Center - MEC
MEC offers Adult Enrichment Programming and Youth ADL (Activities of Daily Living) programs. Activities include, but not limited to meals-on-wheels and music classes.

Family Support Funding

Family Care Program
Program Offerings
InCommunity provides temporary respite relief to support families or caregivers of individuals with I/DD in the comfort of their own home. Hours are flexible and available during the week and weekends.

Employability
EmployABILITY provides Job-Seeking and Employment Support for Adults with Developmental Disabilities through Supported Employment (SEI) Services. We offer traditional Job Development, Job Coaching, and Long-term Job Supports and Maintenance services using an individualized, person-centered method.

Individuals diagnosed with a developmental disability.
Post-High School (Graduate, Certificate, or Aged-out)
Qualified to work in the United States.
Desire to work and be part of the community.

Residential Living Application Process
1. Give InCommunity I/DD access through your Support Coordinator
2. InCommunity Clinical Evaluation Team determines if the requested service area can meet the needs of the individual.
3. An Intake Packet is mailed if services are recognized as a good fit.
4. Once approved, a move in date or start date is identified via phone or email.

Day Programs Application Process
1. The Family must call to set up a pre screening tour with the preferred location.
2. Day Programs will notify the Intake Department of completed prescreening by submitting inquiry forms.
3. The Intake Department contacts the Support Coordinator to request I/DD access for review.
4. I/DD information is reviewed by the Clinical Evaluation Team.
5. If accepted, an Intake Application is mailed to the Family.
6. A start date is issued once Day Program Managers have a complete Intake Application, Prior Authorization and Community Access Group Goals.

Key Terms
Intellectual Disability
Intellectual disability refers to neurodevelopmental conditions that affect functioning in two areas:
Cognitive functioning, such as learning, problem solving and judgment.
Adaptive functioning, activities of daily life such as communication skills and social participation.
Additionally, the intellectual and adaptive deficit begin early in the developmental period, prior to the age of 18 years for diagnosis.

Developmental Disability
Developmental disability is the broad term used to describe a range of conditions that affect physical and/or mental functioning. For a condition to be considered a developmental disability, the manifestations or symptoms of the condition must be present in childhood years and expected to be present for life. Most developmental disabilities are present at birth and may be identified early in life, although some developmental conditions may not be recognized until after age three. Conditions that arise or are detected in adolescent or adult years are not considered developmental disabilities.

Respite Care
Respite care is the provision of short-term, temporary relief to those who are caring for family members who might otherwise require permanent placement in a facility outside the home.
NOW/Comp Waiver
New Option Waiver Program (NOW) and Comprehensive Support Waiver Program (COMP). A Medicaid waiver is money that may be used to pay for services for a person with intellectual, developmental, or physical disabilities. These services can take place in the person’s home or in the community.
NOW and COMP are for eligible people with intellectual and/or developmental disabilities who require an intermediate care facility.
NOW offers services and support to enable individuals to remain living on their own or in their family home and participate in the community.
COMP serves individuals with more intensive needs, primarily those with intellectual and/or developmental disabilities who require residential care.
NOW/COMP programs are accessed through six regional DBHDD field offices.
